I think the idea is that by killing the Doctor, River kills everyone he would have saved in the rest of his lives, across all time and space. Conceivably this could be “worse than Hitler” within the fictional universe of Doctor Who. But if this is what they mean, they don’t explain it. Also, since we live in the real world and not the world of Doctor Who, it is a rather insensitive remark, yes.

That’s the main thing that bugs me. Hitler didn’t even have to be in it, either. Least favourite episode, woe. :(
