Previously, I’d only seen the first two panels and assumed it was the complete comic.

I don’t know… as someone with a pretty disabling condition, I’m not sure I like the idea of a world with no supports or accommodations.
I think this is more in reference to manmade systemic barriers, like economic inequality, than it is situations out of our control, like disabilities. In the latter case, there is no general societal barrier that could be affected that would remove the need for accommodations, we can remove the stigma, but people like you and I will still be ill and need what we need.
